Immunization Requirements

Student Requirements
The  North Dakota University System policy requires:

  1. All students to be vaccinated with two doses of measles, mumps, and rubella.
  2. All newly admitted students ages 21 and younger must provide documentation of a meningitis vaccination (MCV4, Menveo, or Menactra) given after the 16th birthday. Meningococcal B and Meningococcal C vaccine such as Trumenba and Bexsero do not fulfill the requirement.
  3. All Students are also required to complete the Tuberculosis (TB) Screening Questionnaire Form.
  4. All Students under the age of 18 years must have a parent or guardian complete the Parent Consent for Minor form to receive services at MSU Student Health.
Minot State University is not requiring COVID-19 vaccinations for faculty, staff or students. Some academic program affiliates (clinical sites, lab sites, etc.) may require vaccination.

Tuberculosis Screening
If the Student Health Center determines that you are at high-risk for tuberculosis exposure, you will need to get a tuberculosis blood test done at MSU Student Health Center upon your arrival to campus. Student Health will contact you through your campus e-mail if you are required to have testing. Testing done outside the United States is not accepted.

Submit Your Health Records
To submit immunization, tuberculosis documentation and other health records:

  1. GO TO MINOT STATE STUDENT PORTAL - If you have claimed your Minot State account, you may access the Minot State Student Health Portal, use your Campus Connection user name and password to log in.

  2. UPLOAD IMMUNIZATIONS
    • Go to Upload > Choose Document You Are Uploading > Immunization Record > Select File > Upload

  3. TUBERCULOSIS SCREENING QUESTIONNAIRE
    • Go to Electronic and Downloadable Forms > Student Health Clinic > Health-Tuberculosis (TB) Screening Questionnaire > Complete and submit

  4. PRINT AND UPLOAD PARENT CONSENT FOR MINOR. If you are under the age of 18 years, your parent or guardian must complete a Parent Consent for Minor form to receive services at Minot State Student Health.
    • Go to Electronic and Downloadable Forms > Forms > Parent Consent for Minor > Print > Complete the form > Go to Upload > Choose Document You Are Uploading > Parent Consent for Minot > Upload

The mandatory insurance coverage for international students through United Healthcare covers the cost of immunizations and tuberculosis screening for non-Canadian international students.

Due Dates
Documentation must be provided by August 1st for Fall Semester, January 1st for Spring Semester, and May 1st for Summer Semester. Failure to comply with the immunization requirements will result in a hold on your account restricting you from registering for the following semester. PLEASE NOTE: The required documentation must be submitted through the MSU Student Health Portal, Student Health will no longer accept paper documentation submitted via fax, e-mail, mail, or hand delivery. Once you have uploaded your documentation, student health nurses will verify that you are compliant with the requirements.

How to submit immunization, tuberculosis documentation and other health records:

  1. GO TO MSU STUDENT PORTAL-  If you have claimed your MSU account, you may access the MSU Student Health Portal, use your Campus Connection user name and password to log in. Click here for instructions to claim your MSU account if you have not claimed your account.
  1. UPLOAD IMMUNIZATIONS
    • Go to Upload > Choose Document You Are Uploading > Immunization Record > Select File > Upload
  1. TUBERCULOSIS SCREENING QUESTIONNAIRE
    • Go to Electronic and Downloadable Forms > Student Health Clinic > # 4-Health-Tuberculosis (TB) Screening Questionnaire > Complete and submit

Immunization Exemptions
Immunization exemptions are allowed for the following reasons:

  • Students who are taking courses off campus, such as on-line, correspondence, etc.
  • Immunization contraindicated by medical condition
  • Student has had one immunization and agrees to have second immunization within one month
  • A student’s beliefs preclude participation in an immunization program

If you have medical or religious reasons for not receiving the required immunizations, please complete the Medical/Conscientious Exemption section of the Mandatory Immunization and TB Screening Form.  A physician's signature is required for a Medical Exemption.   PLEASE NOTE: By requesting the exemption to immunization, the student may be excluded from all campus activities, including classes, in the event that the North Dakota Department of Health declares the existence of a measles, mumps, rubella or meningitis outbreak at the University.  This exclusion shall remain in effect for such time as determined by the North Dakota Department of Health.

  2. Go to Electronic and Downloadable Forms > Forms > Immunization Exemption Form > Print > Complete the form > Go to Upload > Choose Document You Are Uploading > Immunization Exemption Form > Upload

Where can I get immunizations, immune titers, and TB testing?
TB testing, immune titers, MMR, Meningitis and influenza vaccines are available at the Student Health Center. You can schedule an appointment by calling the clinic at 701-858-3371.

All vaccines and TB testing are also available at First District Health Unit (701-852-1376) and medical centers. Cost varies depending on your insurance. 

How do I know if I am compliant?
Go to the MSU Student Health Portal (Use your campus connection user name and password to log in) > Immunizations > Print. Click here for instructions to claim your MSU account.

On this page under status, it will say compliant or not compliant. Review your record to determine what requirements you are missing. Student Health will send you a reminder when you are due for an immunization, TB testing, or if you are not in compliance. Messages will be sent through a secure message to your email.  Please do not ignore these e-mails. Failure to take action with these reminders may cause a student HOLD. If you have questions, call Student Health at 701-858-3371.

How do I claim my student account in Campus Connection?
If you have not already done so, you must activate your student account in Campus Connection. To claim your account:

  1. Go to MinotStateU.edu/cc
  2. Click “Claim Your Account”
  3. Answer the appropriate security questions
  4. Enter your DOB and your student ID number (also known as your EMPLID), which was included in your acceptance letter
  5. Follow the steps as indicated
  6. You will then obtain a login and password for Campus Connection

How do I get a copy of my immunization and TB records from Student Health?
You can obtain a copy of your immunization records and TB results by going to the MSU Student Health Portal (Use your campus connection user name and password to log in) > Immunizations > Print. If it shows that you have no records on the MSU Student Portal, go to Electronic and Downloadable Forms  > Forms > Release of Medical Information > Print > Complete > Upload > Choose document you are uploading > Release of Medical Information > select file > submit. Allow 7-10 business days.

If you are no longer an active student and do not have a campus connection user name or password, please print and fax the Release of Medical Information Form (pdf) to 701-858-3997.

How do I get my Student Health Hold removed?
Student that are not in compliance with the NDUS Immunization and TB Policy will have a hold placed on their student account restricting you from registering for the following semester. The holds can be removed easily by submitting the required documentation. Allow 24 hours for the hold to be removed once you have uploaded your information, call 701-858-3371 for immediate assistance. Follow these instructions to upload your documents:

  1. GO TO MSU STUDENT PORTAL-  If you have claimed your MSU account, you may access the MSU Student Health Portal, use your Campus Connection user name and password to log in). Click here for instructions to claim your MSU account if you have not claimed your account.

  2. UPLOAD IMMUNIZATIONS
    • Go to Upload > Choose Document You Are Uploading > Immunization Record > Select File > Upload

TB testing, immune titers, and all required vaccines, EXCEPT varicella vaccine, are available at the Student Health Center. You can schedule an appointment through the MSU Student Health Portal (Use your campus connection user name and password to log in).

All vaccines and TB testing are also available at First District Health Unit - 852-1376. Cost varies depending on your insurance.
